Forms for F-1 Students

Document Request Form - For F-1 requests to ISS (travel signature, letters of enrollment, etc.)

  • New March 2016: Submit your request for travel signature pre-approval and letters (Verification of Immigration Status, Invitation and Expense) online through UA International

  • New May 2016: Submit your request for a replacement I-20 online through UA International (under Other)

  • New November 2016: Submit your request for a Change of Major I-20 online through UA International (under Academic Changes)

Financial Guarantee Form - For requests that require proof of funding (for cost of attendance click here)

Concurrent Enrollment Form - For approval to take classes at Pima Community College

Full-Time Enrollment Exemption Form - For approval to enroll less-than full-time in your final semester at the UA

Graduate Coursework Completion Form - For students who have completed coursework and only have 900-level thesis/dissertation units remaining

UA Employer SSN Letter - For students with on-campus employment requesting an SSN (must be printed on departmental letterhead)

  • New May 2016: Upload your UA Employer SSN Letter to the Social Security Letter e-form in UA International to request a SSN letter (under F-1 Student Employment).

New May 2016: Submit your request for an I-20 recommending the STEM OPT Extension through the STEM OPT Extension e-form in UA International (under F-1 Student Employment)

OPT Reporting E-Form - For reporting your employment and address while on OPT

Cap-Gap Extension E-Form - For students who are beneficiaries of a timely filed H-1B extension but whose F-1 status (OPT or the 60-day grace period following the end of OPT) ends between April 1 and October 1 of the current year

Transfer Out E-Form - For transferring your SEVIS record to a new university

Employment Authorization Form - For requesting permission for on-campus employment (only UA-issued DS-2019s)

  • New May 2016: Upload your UA Employer SSN Letter and Employment Authorization Form to the Social Security Letter e-form in UA International to request a SSN letter (under J-1 Student)

Academic Training Packet - For applying for off-campus work authorization (appointment with ISS advisor required).

J-2 Work Authorization Packet - For your J-2 dependent to request work authorization from U.S. Citizenship and Immigration Services (USCIS).

How to Ship Your Document

If you are currently outside of Tucson, you can arrange to have your documents mailed to you by FedEx or UPS at your own expense through eShipGlobal.  Please wait until your request has been approved to create a mailing label.

Instructions

These instructions are only for continuing students. Newly admitted students cannot access the e-form through UA International and should go directly to the eShipGlobal website to create an account.

  1. Log into UA International and submit the eShipGlobal e-form.

  2. After you submit the eShipGlobal e-form, you will be directed to the eShipGlobal website. If you have an account with eShipGlobal that is linked to your UA email address, you will be prompted to log in. If you do not have an eShipGlobal account, or you have an eShipGlobal account associated to your personal email address, then you must create a new account with your UA email.

  3. After creating your account or logging in, you will be asked what you would like to do. Select “Receive documents from your University.”

  4. Enter your shipping address and contact information, select the type of express mail service you would like to use, and pay for shipping.

  5. Your mailing label will be sent directly to your UA International record, so ISS staff can download and print the label when your documents are ready to be mailed. You will receive an email from eShipGlobal with the tracking number once your documents have been mailed. You can also log into UA International to check the eShipGlobal e-form status. The e-eform status will be changed to approved once your documents have been shipped.

If you have any questions about eShipGlobal or you need technical support, please email eShipGlobal.  If you do not have access to the eShipGlobal e-form in UA International, and you are not a newly admitted student, please email ISS.
